Please use the same email address that you’ve used with us in the past. Then, visit the Club between 9:00 a.m. and 5:00 p.m. to complete the required supplemental forms listed below.

• Parent/Student Handbook
• Technology Policy
• School Student Release Form

The forms above must be signed in person for your child to be fully registered and to attend the program.

• Permission to Administer Medication Form
• Opt-Out Photo Release Form
